Permission Slip
An email will always be sent out with all pertinent information to be filled out on the permission slip (the troop
information) Normally permission slips will be sent home pre-printed with trip/activity information, this form may be printed
out and filled in if, for example, you were not sure your daughter would be participating until just before the trip/activity.
You will need to do the following: 1. print a copy, 2. fill in information (provided to you in an email for this purpose!)
3. sign, and 4. bring to the event and make sure it is handed to a leader, to keep on file. No permission slip, no paticipation

If you should find you need to update your daughter's health record, (ie: a newly discovered allergy to something,
